In Nevada, the medical marijuana program is regulated by the Nevada Division of Public and Behavioral Health (DPBH) under the Department of Health and Human Services.

Before a person can even apply to become a registered cardholder in the state of Nevada, one must first mail a written request form to the DPBH asking for an application. Requests must be mailed using the U.S. Postal Service, UPS or FedEx only. Walk-ins are not permitted.

How do I request an application?

A patient must submit a written request form to receive an application packet in order to apply for a Medical Marijuana Registry Identification Card in Nevada. The written application request form must be mailed to:

A non-refundable application fee of $25 must be mailed with the application request form. The $25 check or money-order must be made payable to Division of Public and Behavioral Health (DPBH).

The information required for the request includes:

  • The address the application should be mailed to
  • If you have a caregiver, include a request for a caregiver packet (caregiver’s full name and date of birth)
  • If you are requesting an application for another person, you will need the patient’s full name and address
  • If you are requesting an application for a minor, check the box to request a minor release

The state’s official application will then be mailed to the patient’s home within 3 business days of the request being received.

What will my application packet consist of once I receive it?

  1. Application form
  2. Attending physician’s statement (to be completed by doctor)
  3. Notarized patient waiver form
  4. Notarized patient acknowledgement form
  5. Caregiver application form
  6. Proof of Nevada Residency (copy of patient’s state issued driver’s license or identification card)
  7. Notarized minor release form (if patient is under 18 years of age)

How do I apply once my application packet is complete?

Mail all completed documents and a registration fee of $75.00 in one envelope to:

If the patient’s application is found to be complete, the Nevada Medical Marijuana Program will mail a temporary approval letter. This temporary approval letter is valid for thirty days. On the same day the temporary approval letter is mailed, the NMMP will conduct a criminal background check on the patient, and the patient’s caregiver (if applicable).

It may take up to fifteen days for the background check to process. If the patient passes the background check, the application will be approved. Approved applicants will receive an acceptance letter. This acceptance letter must be taken to an approved Nevada Department of Motor Vehicles to receive his or her medical card. The cost of the medical marijuana photo identification card is $11.

Once approved, each cardholder is permitted to have up to 2.5 ounces of usable marijuana in a 14 day period. If the patient is cultivating, he or she is permitted to have up to 12 plants at one time. Registration cards must be updated annually.

Qualifying Medical Conditions:

  • AIDS
  • Cachexia
  • Cancer
  • Glaucoma
  • Post-traumatic stress disorder (PTSD)
  • Persistent muscle spasms or seizures
  • Severe nausea or pain
  • Other conditions are subject to approval
