As of August 2014, New Hampshire’s Medical Marijuana Program is not yet accepting applications for patients or alternative treatment centers.

What will qualify a patient to apply for the Medical Marijuana Program?

  • Be a resident of the state of New Hampshire
  • Have a qualifying, debilitating medical condition
  • Obtain written certification from a licensed physician
  • A legal guardian must apply for a patient under the age of 18

Patients will be required to submit the following to apply:

  1. Complete, signed patient application
  2. Complete, signed physician certification
  3. Current passport sized photograph of patient
  4. Proof of residency
  5. Legible copy of patient’s New Hampshire driver’s license or identification card
  6. Non-refundable registration fee
  7. *Any additional documents required for patients under the age of 18

Mail or hand deliver all of these documents in one envelope to:
